Northern Health Travel Grant Application (0327-88E)

Northern Ontario residents who travel at least 100 km one way to an OHIP-insured medical specialist or ministry-funded facility use this paper application to claim a travel grant and accommodation allowance. The specialist or facility completes Section 6, confirming the service and declaring the medically necessary lodging nights. A separate application is needed for each round trip, received within 12 months.

Who completes it

The patient, guardian, substitute decision-maker or third-party agency completes Sections 1 to 5, including companion and payment details. The specialist or ministry-funded facility provider must complete and sign Section 6, the mandatory Speciality-Service Provider Information.

Submitting and payment

How to submit it
The patient mails the paper application with original receipts to the Ministry of Health's NHTG Program within 12 months of the date of service. Patients applying online instead attach the separate Specialty-Service Provider Form (ON00857).
Who pays
Section 6 of the form lists physician fee code K036.

Good to know

  • For service dates on or after December 1, 2024, the accommodation allowance depends on the medically necessary lodging nights the provider declares in Section 6, from 1 to 8 or more.
  • Section 6 also asks whether the service is OHIP-insured or WSIB-related, whether the patient was hospitalized, and whether it involves an ADP device or the cleft lip and palate, seniors dental or fertility programs.
  • Qualifying providers include RCPSC-certified specialists, CPSO specialists outside family practice, GPs with a GP Focused Practice designation and ministry-funded facilities.
  • The ministry encourages telemedicine through the Ontario Telemedicine Network where it can replace travel, and the grant does not cover meals or taxis.

Is the NHTG application available in French?

Yes. The French edition, 0327-88F, is titled Demande de subvention accordée aux résidents du Nord de l'Ontario pour frais de transport à des fins médicales. Both versions are posted on Ontario's central forms repository, and the English form notes that it is available in French.

How long does the patient have to submit an NHTG application?

The Ministry of Health must receive the application within 12 months of the date of service, and late applications are not considered. Requests for reconsideration must arrive within 12 months of payment, denial or return. The ministry pays by cheque within about six weeks of a correctly completed application.
