SaskAbilities Accessible Parking Permit Application

SaskAbilities runs Saskatchewan's Accessible Parking Program, and this two-page form covers new permits, renewals and replacements. A healthcare professional completes Section 2 for people unable to walk unassisted more than 50 metres without great difficulty or danger to their health, naming the condition and choosing a short-term, long-term or permanent duration. The form must reach SaskAbilities within three months.

Who completes it

The applicant, parent, guardian or power of attorney completes and signs Section 1. A physician, occupational therapist, physical therapist, nurse practitioner or chiropractor completes and signs Section 2, the medical information.

Submitting and payment

How to submit it
Mail the original form to SaskAbilities in Saskatoon or bring it to any SaskAbilities location within 3 months of the healthcare professional completing it. Email and fax are not accepted because original signatures are required.
Who pays
The applicant is responsible for any healthcare professional's charge for completing the form and pays SaskAbilities a non-refundable permit fee.

Good to know

  • Short-term permits cover 1 to 12 months, long-term permits cover conditions that may improve over the next 3 years, and permanent holders renew by self-declaration without a new medical section.
  • For non-visible disabilities, such as COPD or cardiovascular disease, Section 2 asks the professional to specify the risk to health of walking 50 metres.
  • Medical information that is visibly altered may lead to rejection, and the certifying professional may be asked to verify the disability if the permit is misused.
  • Information on the application may be shared with SGI for audit purposes, and the permit may only be used when the holder is entering or exiting the vehicle.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

Vero is not affiliated with, or endorsed by, the agencies and insurers listed here. Read our editorial policy.

Frequently asked questions

Who can sign the Saskatchewan accessible parking permit application?

A physician, occupational therapist, physical therapist, nurse practitioner or chiropractor. They name the disabling conditions, describe in plain language how mobility is impaired, choose the duration and certify that the applicant is eligible under the 50-metre criterion, with their name and address printed or stamped.

Can the SaskAbilities parking form be emailed or faxed?

No. SaskAbilities requires the original signatures of both the applicant and the healthcare professional, so emailed or faxed forms are not accepted. The form is mailed to the Saskatoon office or brought in person to a location in Saskatoon, Regina, Prince Albert, Swift Current or Yorkton.

Does a permanent parking permit need a new medical form at renewal?

No. When the professional marks the disability as permanent, the holder can renew by self-declaration in Section 1 without verification from a healthcare professional. Short-term and long-term permits need a new Section 2 to renew or extend.
