Saskatoon Access Transit Application Part B

Saskatoon's Access Transit, a shared-ride specialized service for people whose temporary or permanent disability restricts use of regular buses, needs this external assessment alongside the applicant's Part A. A health care practitioner familiar with the disability describes objective restrictions, how they vary by season or time of day, and the expected duration of need. Applications are not considered until both parts arrive.

Who completes it

A registered nurse, nurse practitioner, registered psychiatric nurse, licensed practical nurse, occupational therapist, physical therapist, rehabilitation therapist, optometrist or physician familiar with the applicant's disability completes and signs all of Part B. The applicant or a designate completes Part A.

Submitting and payment

How to submit it
Send Part B to Access Transit by mail, email or fax, without faxing the instruction page. Applications are not considered until both Part A and Part B are received.
Who pays
Any charges for completing Part B or obtaining additional information are the applicant's responsibility.

Good to know

  • Question 8 asks whether the condition prevents or makes difficult boarding or sitting on buses, understanding schedules or reaching stops, and Question 9 whether travel training would help.
  • A mandatory attendant travels on every trip without paying a fare; Part B asks whether the applicant needs one for medical or behavioural safety reasons.
  • Question 13 sets the expected need: less than 6 months, 6 to 12 months, temporarily up to 2 years, or permanently.
  • Incomplete or unclear forms are returned, and completing Part B does not guarantee eligibility; Access Transit may ask the practitioner for more information.

Frequently asked questions

Who can complete Part B of the Saskatoon Access Transit application?

A health care practitioner familiar with the applicant's disability: registered nurse, registered nurse practitioner, registered psychiatric nurse, licensed practical nurse, occupational therapist, physical therapist, rehabilitation therapist, optometrist or physician. The practitioner records their specialty, relationship to the applicant and how long they have been involved in assessing the condition.

Does the practitioner need to do a functional assessment?

Part B asks whether you completed a functional ability assessment or a cognitive assessment during the appointment and, if so, to explain it and give the date. It also asks for the applicant's current treatment plan and whether their restrictions are expected to change over time.

What restrictions does Part B ask the practitioner to describe?

Objective medical restrictions in walking, mobility, strength and endurance, vision and perception, memory and cognition, behaviours affecting travel, and other health concerns such as diabetes, obesity or angina. The practitioner also notes whether restrictions vary by season or time of day, for example at night or in winter.
