Alberta AB-3 Progress Report: Auto Injury Form

Alberta's primary health care practitioners use this report to update a claimant's auto insurer on recovery after a motor vehicle collision. It is completed at the insurer's request and rates progress on up to three functional goals, from regressed to resolved. The form applies to accidents on or after October 1, 2004 and is forwarded to the insurance company.

Who completes it

The primary health care practitioner completes Part 2 (practitioner information) and Part 3 (Therapy Status Report), then signs Part 4. Part 1 holds the claimant's details, and a header section records the insurance company, policy number, claims representative and claim number.

Submitting and payment

How to submit it
The AB-3 directs that it be forwarded to the insurance company. It is a dynamic Adobe LiveCycle (XFA) PDF, so open it in Adobe Reader rather than a browser.
Who pays
For the AB-3, fees in the Superintendent of Insurance bulletins apply only to DTPR claims; under the AAIBR, Alberta says expenses for completing AB claims forms are not prescribed and must be reasonable.

Good to know

  • Each functional goal is rated as regressed, improved minimally, moderately or significantly, plateaued, resolved or other, with comments on limitations and any further treatment required.
  • Part 3 also records the diagnosis, key subjective, physical examination or functional questionnaire findings, and the number of treatments to date with the treatment provided.
  • The AB-3 follows the AB-2 Treatment Plan; at discharge, the AB-4 Concluding Report summarizes treatment and the reason for discharge.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

Frequently asked questions

When do I complete an Alberta AB-3?

Alberta lists the AB-3 as completed by the primary health care practitioner at the request of the insurer. It reports progress since the AB-2 Treatment Plan, including treatments to date and status on each functional goal. Send the completed form to the insurance company named in its header section.

Who signs the AB-3 Progress Report?

The primary health care practitioner signs Part 4, certifying that the information is true and correct to the best of their knowledge. The form lists medical doctor, chiropractor and physical therapist among the professions. A digital signature locks the form above the signature line, so make final edits before signing.
