WorkSafeBC forms for BC physicians, NPs and therapists

Physician, nurse practitioner, chiropractor, physiotherapy and contracted-program reports for WorkSafeBC claims, with deadlines and how each one is submitted.

WorkSafeBC handles workers' compensation claims in British Columbia. Physicians report on the Physician's Report (Form 8/11): Form 8 after the first visit and Form 11 for progress on the same claim. Nurse practitioners use Form 8NP/11NP and chiropractors Form 8C/11C. Contracted musculoskeletal physiotherapists start with the Physiotherapy Treatment/Post-Surgical Initial Report (83D555). Psychologists, occupational therapists and other providers in contracted programs use each program's report templates.

Send Form 8/11 through Teleplan when you can: it is the preferred channel and pays the higher rate. Payment drops for late reports, and nothing is paid after six business days. Contracted-program reports are mostly Word templates sent through the Provider Portal. Before operating, surgeons request approval on the Authorization Request for Surgery (83D6), faxed with the consult report. All WorkSafeBC forms in this library are in English. Vero Documents fills WorkSafeBC's PDF forms from the visit note and the chart.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

Frequently asked questions

What is the difference between WorkSafeBC Form 8 and Form 11?

They share one Physician's Report (Form 8/11). Form 8 is the first report after the first visit, and only one is allowed per claim. Form 11 is the progress report. Send it when the condition, treatment or return-to-work status changes, when recovery is slower than the norms, or when more than four weeks have passed since the last Form 11. Extra clinical detail goes on the Form 8/11 Addendum.

When is WorkSafeBC Form 8 due?

Within three business days of the first visit if the worker may need time off beyond the day of injury. The same deadline applies to claims for a back injury, hernia, shoulder or knee strain or sprain, occupational disease or mental disorder. For the highest rate, WorkSafeBC must receive Form 8 within one business day and Form 11 within three. Nothing is paid for reports after six business days.

Which report does a nurse practitioner send to WorkSafeBC?

The Nurse Practitioner's Report (Form 8NP/11NP): 8NP as the first report and 11NP for progress. 8NP is due within three business days for back injuries, hernias, shoulder or knee strains and sprains, occupational disease or mental disorders, or when WorkSafeBC asks. Send 11NP when recovery is off track, disability runs past the usual norms, or the patient is ready for return-to-work help. WorkSafeBC pays under its Nurse Practitioner Fee Schedule.

What do chiropractors submit to WorkSafeBC?

The Chiropractor's Report (Form 8C/11C). Submit and invoice the 8C through Teleplan within three days of the initial visit. Send the 11C discharge report through My Provider Services within seven days of the last visit. Treatment extension requests go on the Chiropractic Treatment Report (83D667), due at week 6 or treatment 8, whichever comes first. Relevant X-ray findings go on Form 11RC.
