WorkSafeBC Form 83D56: Requisition for Medical Imaging

Physicians use this three-page requisition to order ultrasound, CT, MRI or interventional procedures for a worker with a WorkSafeBC claim, and WorkSafeBC's specialist requirements page lists it as the diagnostic imaging referral form. It records the exact examination, prior imaging, medical history, contrast and MRI safety screening, anticoagulant details for interventional procedures, and where copies of the report go.

Who completes it

The ordering physician, whether specialist or primary care, completes the requisition in ink and signs it with their MSP number.

Submitting and payment

How to submit it
Complete it in ink; the form directs that a copy of the imaging report be faxed to WorkSafeBC, listed first under copies of report. Incomplete requisitions are returned.

Good to know

  • MRI lumbar spine for adults needs at least one criterion, such as significant or progressive neurological deficit, infection or tumour risk factors, or radicular symptoms lasting over 12 weeks.
  • For MRI knee or hip in patients 40 and older, eligibility requires suspected infection, osteonecrosis, suspected tumour or a fixed locked knee.
  • For MRI, report any welding, grinding or metal in the eye (an orbital X-ray report is needed first), implanted devices, and claustrophobia.
  • Suspected cauda equina syndrome or a significant acute traumatic event just before symptoms began should be sent to the emergency department instead.

Frequently asked questions

When does WorkSafeBC consider an MRI of the lumbar spine appropriate?

The requisition's criteria require at least one of: significant or progressive neurological deficits, risk factors for infection or tumour, radicular pain or neurogenic claudication lasting over 12 weeks, previous lumbar surgery with another criterion met, or an MRI recommended on a previous imaging report. MRI is not indicated for acute mechanical low back pain.

Is an MRI needed before a specialist referral for knee or hip pain?

Not always. For patients 40 and older, the form says MRI mainly serves surgical planning and adds little in moderate to severe osteoarthritis, so a weight-bearing X-ray is recommended. If the X-ray shows mild or no osteoarthritis, a specialist referral is recommended and a pre-consult MRI is not required.
