WorkSafeBC Form 8/11 Addendum: Additional Information

When clinical details do not fit on a Physician's Report (Form 8/11) or a Chiropractor's Report (Form 8C/11C), WorkSafeBC provides this continuation sheet. Both main forms point to it in their footer, noting that additional information can be recorded on the Form 8/11 Addendum. Each page carries the worker's identifiers so the extra pages can be matched to the claim.

Who completes it

The physician or chiropractor who completed the main report fills in the addendum, starting with the worker's last and first name, WorkSafeBC claim number and personal health number. The rest of each page is open space for the additional information.

Submitting and payment

How to submit it
The addendum is a fillable PDF, and it directs completed paper practitioner reports to WorkSafeBC by fax or by mail.
Who pays
The addendum lists no fee item of its own; WorkSafeBC pays for the Form 8/11 or 8C/11C report itself under the physician or chiropractic fee schedules.

Good to know

  • The Physician's Report (Form 8/11) footer says additional information can be recorded on the Form 8/11 Addendum, and the Chiropractor's Report (8C/11C) carries the same note.
  • Each page repeats the worker's name, claim number and personal health number, so a separated page can still be matched to the right claim.
  • WorkSafeBC also lists it as Medical Services: Practitioners Report/Additional Information, published December 2008, with revision R12/08 printed on the form.
  • Don't let the addendum hold up the main report: WorkSafeBC pays physician reports on a sliding scale and only if received within six business days.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

Frequently asked questions

When should I use the Form 8/11 Addendum?

Use it when the clinical information, history or return-to-work details you need to report do not fit in the boxes of the Physician's Report (Form 8/11) or the Chiropractor's Report (Form 8C/11C). Both forms print a reminder that additional information can be recorded on the addendum.

Can chiropractors use the 8/11 Addendum?

Yes. The PDF's own description calls it an addendum to forms 8/11 and 8C/11C, and the Chiropractor's Report footer points to it for additional information. The page header reads Practitioner's Report, Additional Information, so it is not limited to physicians.

How quickly must the Form 8/11 report reach WorkSafeBC?

Physicians should send Form 8 within three business days of the first visit. Form 8 reports received within one business day and Form 11 reports within three business days are paid at the highest rate, rates fall each day after that, and reports must arrive within six business days to be paid.
