WorkSafeBC Contracted Rehab Program Report Forms

WorkSafeBC lists 32 rehabilitation programs and services, including the Comprehensive Mental Health Program, Occupational Therapy Services, Functional Capacity Evaluation, hand therapy, concussion, pain and medication management and hearing aid services. Providers in these networks hold long-term contracts, and each program has its own report templates, mostly Word documents, with requirements set in the contract and program reference manual.

Who completes it

Contracted providers complete the report for their own program: registered psychologists, clinical counsellors and clinical social workers in the mental health program, occupational therapists in OT Services, FCE assessors, audiologists and hearing aid providers, and other program clinicians.

Submitting and payment

How to submit it
Most contracted programs use WorkSafeBC's secure Provider Portal to receive referrals and submit reports and invoices. Hearing aid providers invoice through My Provider Services and send replacement requests through the claims document uploader.
Who pays
WorkSafeBC pays contracted providers under each program's services agreement and fee schedule, and only for services authorized in the referral.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

Frequently asked questions

How do I become a WorkSafeBC contracted rehabilitation provider?

Program contracts come up for bidding every few years, and you apply when the bidding process begins. WorkSafeBC posts notices through its Bid Opportunities page, with BC Bid alerts available for some programs. Once you have a contract, Procurement Services assigns a payee number that you use in all communications with WorkSafeBC.

Where are the reporting rules for each program?

WorkSafeBC says everything needed to complete the required paperwork is in the contract documents you sign. Programs such as the Comprehensive Mental Health Program also publish a reference manual covering program requirements, referral processes, service delivery expectations, reporting requirements and links to the required forms.

Does WorkSafeBC pay for services outside the referral?

In the Comprehensive Mental Health Program, treatment is payable only when authorized under the claim. Authorization arrives as a referral through the Provider Portal listing the approved services, and missed appointments and services not listed in the referral letter are not covered.
