WorkSafeBC 83D599: Concussion Physio Assessment Report

Physiotherapists contracted with WorkSafeBC for concussion and vestibular physiotherapy (CVPT) use this seven-page report for the initial assessment or a reassessment. It records concussion criteria based on the American Congress of Rehabilitation Medicine, symptoms and history, outcome measures, neurological and cervical screens, a vestibular ocular-motor screen, balance testing, job match, and the treatment goals, visits and expected outcome.

Who completes it

The contracted CVPT physical therapist completes the whole report, including attempts to contact the employer, and records whether the plan has been communicated to the claim owner.

Submitting and payment

How to submit it
Upload it through WorkSafeBC's claims uploader if you bill in Teleplan, or with the invoice in My Provider Services if you bill there; the form also lists fax and mail.
Who pays
WorkSafeBC pays under its CVPT fee schedule, and the date of service on the invoice must match the assessment or reassessment date on the form.

Good to know

  • Record up to two attempts to contact the employer, the job attachment status, and whether light or modified duties are confirmed as available.
  • The vestibular ocular-motor screen rates headache, dizziness, nausea and fogginess from 0 to 10 for smooth pursuits, saccades, convergence, VOR and visual motion sensitivity.
  • Include at least one outcome measure, and list up to four recovery and return-to-work factors ranked from most to least critical.
  • Later reports use the CVPT Extension Request (83D600) and the CVPT Discharge Report (83D601), which is due within 7 days of the last visit.

Frequently asked questions

What concussion criteria does WorkSafeBC's CVPT assessment use?

The subjective findings table is based on the American Congress of Rehabilitation Medicine criteria for concussion. It asks whether there was loss of consciousness (and whether it was witnessed), memory loss for events immediately before or after the injury, altered mental state at the time, and any focal neurological deficit.

Can a worker in CVPT treatment keep working?

The report asks whether regular or modified duties can be performed alongside physiotherapy based on current functional abilities and, if not, when they could begin. It matches each essential job duty against the worker's measured functional ability and asks for recommended modified hours or duties.
