WorkSafeBC Form 83D346: Physiotherapy Hospital Report

Hospital physiotherapy departments use this two-page report to tell WorkSafeBC about a worker's assessment or reassessment. It covers the accepted injury areas, employer and job attachment, subjective and objective findings, the worker's five most significant barriers to return to work with a job-match check against critical job demands, and a treatment plan with expected outcome and discharge date.

Who completes it

The hospital physical therapist completes the report and is named as the contact, with the hospital's name, payee number and department phone and fax numbers.

Submitting and payment

How to submit it
Send it to WorkSafeBC by fax or mail using the Claims Call Centre details printed in the form's footer.
Who pays
Payment goes to the hospital's WorkSafeBC payee number, and fields marked with an asterisk, such as the date of service, must be completed for payment to be processed.

Good to know

  • The current form is revision 17/01, a Word document listed on WorkSafeBC's Hospitals provider page alongside the Hospital Account (Form 11H) and Health Authority Fee Schedule.
  • For each critical job demand, record the worker's current ability and tick whether the two match.
  • Say whether regular or modified duties can be done alongside physiotherapy; if modified duties are not yet possible, give a date they could begin.
  • Expected outcome is ticked as return to full duties and hours, return to modified duties or hours, or other, and the form asks whether the plan has been shared with the claim owner.
