WorkSafeBC Form 83D76: FCE or Cognitive FCE Report

WorkSafeBC may refer a worker whose injury affects their ability to perform work tasks to its contracted Functional Capacity Evaluation network, and the assessor reports results on this Word template. It covers 1-day and 2-day FCEs, functional screen evaluations, 1-day and 2-day Cognitive FCEs and incomplete assessments. Each tested demand is rated against the worker's job as a match, no match or inconclusive.

Who completes it

The contracted FCE or Cognitive FCE assessor completes the whole report and records their name and professional designation(s), while the provider company adds its payee number and contact details. The referral questions it must answer come from the WorkSafeBC officer.

Submitting and payment

How to submit it
Contracted FCE providers submit reports and invoices through WorkSafeBC's secure Provider Portal, and the report is due within 5 business days of completing the assessment. At minimum, a copy goes to WorkSafeBC and to the worker's primary care provider.
Who pays
WorkSafeBC pays contracted providers under their FCE contract, and a complex FCE or Cognitive FCE fee is requested separately on Form 83B545.

Good to know

  • List each referral question from the WorkSafeBC officer with a number, then answer every one in the response section using the matching number.
  • Delete the cognitive demands table for a standard FCE; for a Cognitive FCE it covers attention, memory, planning, judgment, multi-tasking, literacy, numeracy, computer skills and work simulations.
  • The summary of effort asks whether results likely represent the worker's current functional abilities, with comments on consistency, reliability and any inconclusive measures.
  • Give treatment, equipment or work aid recommendations only if the WorkSafeBC officer specifically asked; return-to-work considerations such as pacing, workstation changes or a graduated return belong in the report.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. Frequently asked questions

When is the WorkSafeBC FCE report due?

The template states that the report is due within 5 business days of completing the assessment. It also records the date the referral was acknowledged, the Day 1 assessment date, the completion date and the start and end times of each assessment day, and asks whether a job demands analysis was done with a site visit.

Is Form 83D76 used for both physical and cognitive FCEs?

Yes. One template covers standard and Cognitive FCEs. Strength and physical demand rows, such as lifts, carries, grip strength, reaching, kneeling and walking, apply to both, and the cognitive demands table is kept only for a Cognitive FCE, which WorkSafeBC describes as evaluating cognitive and physical abilities.

Who receives a copy of the FCE report?

The Copies to section notes that, at minimum, a copy should always go to WorkSafeBC and to the primary care provider, such as the attending physician or nurse practitioner. Other recipients can be added. Detailed test data, including trials, repetitions, weights, increments and forces, goes in the appendix.
