WorkSafeBC 83D555: Physiotherapy Initial Report

Physiotherapists contracted with WorkSafeBC for musculoskeletal care complete this four-page report after the initial visit, choosing the standard treatment report (83D218) or the post-surgical report (83D220) when the visit is within 60 days of surgery. It records findings, clinical impression, functional abilities, treatment goals, outcome measures and whether the findings were discussed with the worker.

Who completes it

The physical therapist who saw the worker completes the report, entering the clinic's name, contact details and WorkSafeBC payee number.

Submitting and payment

How to submit it
Upload it within 7 calendar days of the initial visit through WorkSafeBC's online claims uploader if you invoice in Teleplan, or with the invoice in My Provider Services if you bill there.
Who pays
Submitting the report is required for WorkSafeBC to pay the initial assessment under its standard or post-surgical physiotherapy fee schedule, and the invoice date of service must match the form.
