A naturopathic physician treating a worker for a compensable injury, disease or mental disorder sends this first report to WorkSafeBC. It is required only when the worker will be disabled beyond the day of injury, when the claim is for a hernia, back condition, shoulder or knee strain or sprain, or occupational disease, or when WorkSafeBC has requested it.
The naturopathic physician completes and personally signs the whole report, adding payee and practitioner numbers, and enters the worker and employer details. There is no section for the worker or the employer to complete.

This form isn’t published as a fillable PDF. Vero Scribe writes the visit note it draws on, so the details are ready when you complete it.
A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

Only when the worker will be disabled beyond the day of injury; when the claim is for a hernia, back condition, shoulder or knee strain or sprain, or occupational disease; or when WorkSafeBC has requested it. When it is required, the Workers Compensation Act sets a deadline of 3 days after first attending the worker.
No. There is no formal process or contract. You need to be a registered practitioner with the College of Naturopathic Doctors of B.C. and have a WorkSafeBC payee number, which Procurement Services provides. Check that WorkSafeBC has accepted the worker's claim before beginning treatment.
It asks how the injury happened, for a full examination description specifying right or left, the diagnosis, X-rays taken now or previously, treatment including any procedure, frequency of treatment and the next visit date, and whether the worker should be referred for therapy, specialist consultation, diagnostic imaging or a rehabilitation program.
