WorkSafeBC Form 11N: Naturopathic Progress Report

WorkSafeBC covers up to eight weeks of naturopathic treatment for most compensable injuries, occupational diseases and mental disorders, and this progress or final report is how a naturopathic physician asks to go further. WorkSafeBC reviews the evidence before paying for an extension, and treatment cannot continue until approval arrives. The same form also serves as the discharge report.

Who completes it

The naturopathic physician completes and personally signs the whole form, selecting either Progress Report or Discharge Report and adding payee and practitioner numbers. Worker and employer details are entered by the practitioner, and neither the worker nor the employer signs it.

Submitting and payment

How to submit it
The form says completed reports are to be submitted to WorkSafeBC by fax. The Workers Compensation Act requires a report within 3 days after the worker is able to resume work, with further reports if treatment continues after that.
Who pays
Bill WorkSafeBC on the Naturopathic Physician's Invoice (Form 11AN); naturopathic remedies, treatments and dietary supplements are not paid without prior approval.

Good to know

  • No contract is required, but you must be registered with the College of Naturopathic Doctors of B.C. and hold a WorkSafeBC payee number from Procurement Services.
  • Confirm that WorkSafeBC has accepted the worker's claim before treatment begins; the View Claim Status tool shows the status online.
  • The clinical section asks for the date of service, any discharge date, your final diagnosis, remarks on current condition or discharge status, and time missed from work.
  • Return-to-work planning asks for restrictions, estimated days to modified duties and to full shifts, and an estimated date of maximal medical recovery.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

Frequently asked questions

How long does WorkSafeBC cover naturopathic treatment?

Up to eight weeks for most compensable personal injuries, occupational diseases or mental disorders. Based on a review of the evidence, WorkSafeBC may pay for an extension beyond eight weeks. To request one, submit a Progress Report on Form 11N, and wait for approval before continuing treatment.

Can Form 11N be used as a discharge report?

Yes. The top of the form asks you to select one only: Progress Report or Discharge Report. Its full title is Naturopathic Physician's Progress/Final Report, and the clinical section includes the date you discharged the worker from your care and your final diagnosis in the case.

Which other WorkSafeBC forms go with Form 11N?

The Naturopathic First Report (Form 8N) is sent after the first visit when the reporting criteria are met, and the Naturopathic Physician's Invoice (Form 11AN) is used to bill. All three are posted on WorkSafeBC's naturopathic physicians page, with Form 11N dated December 2008.
