Registered massage therapists send this report to WorkSafeBC after the initial assessment and whenever a WorkSafeBC officer requests one; it also serves as a progress or discharge report. It records time loss, current duties, concurrent physiotherapy or chiropractic care, subjective and objective findings, recovery and return-to-work factors, and the treatment modality, frequency and goals.
The registered massage therapist completes the whole report, adding the clinic's payee number and noting whether there is a physician's referral.

Massage Therapy Treatment Report (Form 83D48)
This form is a Word template, not a PDF. Vero Scribe writes the visit note it draws on, so the details are ready when you complete it.
A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.
Vero is not affiliated with, or endorsed by, the agencies and insurers listed here. Read our editorial policy.
Within 5 business days of the initial assessment, or when a WorkSafeBC officer asks for it. The same form covers WorkSafeBC-requested, progress and discharge reports. Massage therapy is payable only on accepted claims, and the form points to WorkSafeBC's online claim information page for claim status.
An initial assessment plus up to 6 treatments within 8 weeks of the injury need no pre-approval. Treatment beyond 8 weeks requires approval first through the Extension of Massage Therapy Treatment Request (83D516), which should be submitted by the end of week 6 of treatment.
