WorkSafeBC Form 83D556: Consult Report Fax Cover Sheet

WorkSafeBC recommends this one-page cover sheet when a specialist faxes a consultation report on an injured worker. It identifies the consult date, the worker and claim number, and the practitioner and payee numbers, so the report can be matched to the claim. The report itself is the specialist's own narrative, and WorkSafeBC must have it before it will process the invoice.

Who completes it

The specialist or their office staff fills in the cover sheet: date of consult, page count, the worker's name and WorkSafeBC claim number, and the practitioner's name, facility, practitioner number and payee number. The consult report behind it is written by the specialist.

Submitting and payment

How to submit it
Fax the cover sheet with the consult report to WorkSafeBC, since specialist reports must be faxed. Bill separately, through Teleplan or on a Physician's Invoice (Form 11A), within 90 days of the service.
Who pays
WorkSafeBC pays specialist fees under the Physicians and Surgeons' WorkSafeBC Services Agreement and the Doctors of BC / WorkSafeBC Unique Fee Schedule, once it has received the report.

Good to know

  • WorkSafeBC says every consult report needs the report type, the worker's name and claim number, and the date of service or visit; the cover sheet captures all of them.
  • Paper or faxed invoices for MSP fee items do not receive the premium paid on electronic invoices sent through Teleplan, compatible billing software or a billing service bureau.
  • Only surgery needs WorkSafeBC approval: fax the Authorization Request for Surgery (83D6) with the consult report, and request imaging on form 83D56.
  • The cover sheet is a Word document published in March 2022 (revision 21/12); providers outside BC invoice on the Generic Invoice, Medical and Health Care (83D12).

Frequently asked questions

Do specialists have to use the WorkSafeBC consult report fax cover sheet?

WorkSafeBC recommends it rather than requiring it. Its specialist requirements say each consult report must show the report type, the worker's name and claim number, and the date of service or visit, and that reports must be faxed. Form 83D556 collects those details along with the practitioner and payee numbers and the page count.

Will WorkSafeBC pay my consult invoice before it gets the report?

No. WorkSafeBC must receive the report before it can process the invoice, so fax the report promptly. Invoices should reach WorkSafeBC within 90 days of the service date, preferably electronically through Teleplan, since paper or faxed MSP fee items do not get the premium paid on electronic invoices. Invoice status can be checked online.

Does a specialist need WorkSafeBC approval before seeing an injured worker?

Not for trauma care or consultations, including psychiatry. Only surgery requires WorkSafeBC approval: fax the Authorization Request for Surgery (83D6) with the specialist's name and the date, the worker's name and claim number, the proposed surgery and its location, and the consult report.
