WorkSafeBC Form 83D51: Generic Health Care Report

WorkSafeBC asks providers who have no dedicated report form, such as acupuncturists, optometrists, podiatrists and speech-language pathologists, to use this generic report when it requests information. The form captures the report type (initial, progress, discharge or other), provider and worker details, the worker's occupation and current work status, and a free-text clinical report.

Who completes it

The treating provider completes the whole report, entering their WorkSafeBC payee (vendor) number and a mailing address or clinic stamp.

Submitting and payment

How to submit it
Send it to WorkSafeBC by fax or mail using the details printed at the top of the form; acupuncturists must return it within 5 days of WorkSafeBC's request.
Who pays
Services are invoiced on the companion Generic Invoice for medical and health care (Form 83D128), under the provider's WorkSafeBC payee number.
