The ICBC reports BC clinicians complete for injury claims, from the CL489 family physician reports to physiotherapy, counselling, psychology and OT forms.
ICBC, British Columbia's auto insurer, asks treating clinicians for reports on injury claims. Family physicians use the CL489 series: the Standard Medical Report (CL489) or the Extended Medical Report (CL489A) after the initial assessment, then the Reassessment Medical Report (CL489B) when injuries are not recovering as expected or the care plan changes. Consulting physicians use the CL489S. Physiotherapists, chiropractors, counsellors, kinesiologists, psychologists and occupational therapists each have their own CL489 reports.
Physician reports are due within 30 days of the assessment and can go through ICBC's online submission tool, an EMR-integrated version, fax or mail. Allied health providers submit through the Health Care Provider Portal (HCPP), ICBC's preferred route, and most progress reports are completed only when ICBC asks. ICBC pays for reports at set rates, listed on its physicians page and in its Fee Schedule for Health Care Providers. Vero Documents fills PDF forms like these from your visit note and chart, and you review and sign.

Complete one or the other after the initial assessment, never both, and only one report per new claim. Use the Standard Medical Report (CL489) when the injuries did not cause missed work or school or significantly impair daily living. Use the Extended Medical Report (CL489A) when they caused missed or modified work or school, or significant impairment of daily living. Follow-up visits without a report are billed through MSP Teleplan.
Physicians send the CL489, CL489A, CL489B and CL489S through ICBC's online submission tool, an EMR-integrated version, fax or mail, within 30 days of the assessment. Physiotherapists, chiropractors, counsellors, kinesiologists and psychologists use the Health Care Provider Portal (HCPP), or HCPIR if they belong to the ICBC Recovery Network. Approved occupational therapists also use HCPP. Nurse practitioners fax or mail the CL489R with a CL392 invoice or their own.
Not for claims on or after May 1, 2021. The first physiotherapy or chiropractic visit is billed as an Initial Visit instead of a report. The Physiotherapy Progress Report (CL489F) and the Chiropractic Progress Report (CL489D) are completed only when ICBC requests them. Counsellors and psychologists still submit an initial report after their assessment, on the CL489G and CL489P.
ICBC pays physicians a set fee for each report under its own fee codes, for example A94564 for the CL489. The consultation behind a CL489S is billed through MSP Teleplan. Physiotherapists, chiropractors, counsellors, kinesiologists, psychologists and OTs are paid under ICBC's Fee Schedule for Health Care Providers, effective September 27, 2026. Range of motion and scarring reports have their own fees, and ICBC supplies those forms with its request.
