ICBC CL489K: Kinesiology Progress Report

BC kinesiologists use this four-page report to update ICBC on a customer's recovery, but only when an ICBC Claims Contact asks for it. It records work and daily-activity status, initial and current findings, barriers and treatment goals, and an appendix table compares functional testing results with the customer's job or daily-living demands. ICBC pays for it only when complete.

Who completes it

The treating kinesiologist completes the whole report, including the functional abilities appendix, and certifies it. Instead of a diagnosis section, the form asks for the name and profession of the patient's regulated primary health care provider.

Submitting and payment

How to submit it
ICBC Recovery Network kinesiologists submit invoices and reports through HCPIR or the Health Care Provider Portal (HCPP), and kinesiologists outside the network send the report to ICBC by email.
Who pays
ICBC pays the per-report fee in its Fee Schedule for Health Care Providers (CL534), billed under Progress Report in HCPIR, only for reports an ICBC Claims Contact requested.

Good to know

  • Job and ADL demands in the functional abilities table should give specific weights, heights, distances, durations or frequencies, and the form asks whether the patient is medically appropriate for testing.
  • If the patient is off work or on a graduated return, testing focuses on work-related limitations; otherwise it focuses on activities of daily living.
  • Twelve standard kinesiology sessions, each with at least 45 minutes of one-on-one care, are pre-authorized during the Early Access Period, the first 12 weeks after the crash.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

Frequently asked questions

What report templates does ICBC provide for kinesiologists?

ICBC's kinesiologist page lists one report template, the CL489K progress report, marked complete upon request. The kinesiology program guide pre-authorizes 12 standard sessions, including the Initial Visit, in the Early Access Period and requires a Treatment Plan through HCPIR or HCPP when more care is needed.

What happens if a CL489K is incomplete?

ICBC's kinesiology program guide says a progress report must be completed in its entirety before it is submitted and billed. An incomplete report does not qualify for payment, and the kinesiologist must resubmit a completed report to be paid. ICBC also requires the most recently available template.
