ICBC CL489D: Chiropractic Progress Report

BC chiropractors use this four-page report to update ICBC on a customer's recovery from a crash, but only when an ICBC Claims Contact requests it. It compares initial and current findings, records work status, return-to-work planning and activities of daily living, and sets out diagnoses, barriers to recovery and prognosis. ICBC pays for the report only when it is complete.

Who completes it

The treating chiropractor completes the whole report, from invoice and patient details to the recommended care plan, and certifies it. The chiropractor also confirms either that the patient consented to sharing the information with ICBC or that the report responds to an ICBC request under section 28 or 28.1 of the Insurance (Vehicle) Act.

Submitting and payment

How to submit it
ICBC Recovery Network chiropractors submit reports and invoices directly through the Health Care Provider Portal (HCPP) or the HCPIR application, and chiropractors outside the network send the report to ICBC by email. If applicable, select the form's Lock button before submitting, after which the PDF can no longer be edited.
Who pays
ICBC pays the per-report fee in its Fee Schedule for Health Care Providers (CL534), billed under Progress Report in HCPIR, only for a report an ICBC Claims Contact requested.

Good to know

  • Twenty-five standard chiropractic sessions, including the Initial Visit, are pre-authorized in the Early Access Period, the first 12 weeks (84 days) after the crash.
  • There is no chiropractic initial report for claims on or after May 1, 2021; the Initial Visit fee covers the assessment and a standard treatment session.
  • A Treatment Plan must go to ICBC when treatment is requested beyond the Early Access Period or the current approved plan, so one may be needed alongside a requested progress report.
  • Use the most recent template from ICBC's Partners Page; the program guide discourages saving personal versions, and an incomplete report must be completed and resubmitted to be paid.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

Frequently asked questions

When does a chiropractor complete the ICBC CL489D?

Only when an ICBC Claims Contact asks for it. ICBC's chiropractic program guide says progress reports must only be completed on request, must be complete before they are submitted and billed, and do not qualify for payment if incomplete. A completed report must then be resubmitted.

What does the CL489D ask about return to work?

If the patient has not returned to work or is on a graduated return, the report asks whether they can resume pre-accident duties and hours, compares job demands with initial and current findings, and asks whether you support starting a graduated return-to-work plan. If you do, the recovery specialist contacts you to plan it.

Which version of the CL489D should I use?

The current template is CL489D (112022), a four-page fillable PDF that ICBC asks you to download and complete in Acrobat Reader. The chiropractic program guide in effect from September 27, 2026 requires the most recently available template from ICBC's Partners Page.
