ICBC CL489G: Counselling Initial Report

Clinical counsellors treating ICBC customers send this six-page report right after completing the intake assessment. ICBC uses it to assess the client's psychological status after the crash, set a baseline for monitoring recovery, judge whether treatment is reasonable and necessary, and plan around functional limits and barriers. Incomplete reports can delay funding approval or lead to its denial.

Who completes it

The counsellor completes the whole report, including assessment dates, work status, chief complaints, functional status, observed findings, medications and up to five treatment goals. The counsellor certifies it and confirms either client consent or an ICBC request under section 28 or 28.1 of the Insurance (Vehicle) Act.

Submitting and payment

How to submit it
ICBC Recovery Network counsellors submit it with the invoice through the Health Care Provider Portal (HCPP) or the HCPIR application once the intake assessment is complete. Counselling for immediate family members not involved in the crash goes through ICBC's manual billing and document submission process.
Who pays
The initial assessment visit and report fee is capped by the Insurance (Vehicle) Regulation and Enhanced Accident Benefit Regulation and billed once, with any earlier assessment visits billed at the standard treatment fee.

Good to know

  • The Assessment Date(s) field holds four dates, so an assessment can span up to four visits; the visit at which the report is completed is billed at the initial visit and report fee.
  • Suicide Risk and Psychological Assessment Methods are optional sections; ICBC's completion guide says to enter N/A when they are not relevant to the claim.
  • If the client does not consent to sharing the report, ICBC sends the counsellor a CL491 request letter citing the legislation that lets ICBC obtain it.
  • Twelve standard counselling sessions, including the Initial Visit, are pre-authorized during the Early Access Period, the first 12 weeks after the crash.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

Frequently asked questions

When is the ICBC CL489G due?

ICBC's Counsellor Report Completion Guide says the CL489G is provided immediately after the intake assessment is completed, and ICBC's counsellor page says to submit it following completion of the assessment. Duplicate invoices for the same service date may be rejected, so bill each assessment visit once.

What should the Treatment Goals and Plan section include?

Up to five client-centred goals that match the client's symptoms, functional deficits and overall presentation. The plan should reflect crash-related barriers to recovery, explain how the client will benefit and the expected outcomes, and state the number and frequency of treatments requested.

Who qualifies to bill ICBC directly for counselling?

Funding requires meeting the definition of counsellor in the Insurance (Vehicle) Regulation. To bill directly as an Approved Counsellor, ICBC's counselling program guide requires, for example, a master's or doctorate and good standing as a Registered Clinical Counsellor or a Canadian Certified Counsellor.
