This six-page report updates ICBC on a counselling client's progress after the initial report. ICBC may request it when a client is not recovering as expected or new barriers to recovery have been identified, and funds it only when the claim representative asked for it. It records changes in symptoms and function, observed findings and an updated treatment plan.
The treating counsellor completes the whole report and certifies it, confirming client consent or an ICBC request under section 28 or 28.1 of the Insurance (Vehicle) Act.

Counselling Progress Report
Upload the fillable PDF once and Vero Documents fills it from the visit note and the chart, ready for you to review and sign. Download, print, or email the finished form, with the patient’s consent.
A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

You can, but ICBC will not pay for it. ICBC's Counsellor Report Completion Guide says it funds the CL489H only when the ICBC claim representative has requested the report, and will not fund progress reports completed and invoiced without that request.
According to ICBC's completion guide, progress reports may be requested when a client is not recovering as expected or when additional barriers to recovery have been identified. The report gives the claim representative updated findings, functional changes and a revised treatment plan to support funding decisions.
