Psychologists use this six-page report to update ICBC on a customer's progress, but only after an ICBC Claims Contact requests it. It records changes in symptom intensity and functional status since the last assessment, current observed findings, suicide risk, psychological measures, the working diagnosis and updated treatment goals. Incomplete reports do not qualify for payment.
The treating psychologist completes and certifies the whole report, confirming client consent or an ICBC request under section 28 or 28.1 of the Insurance (Vehicle) Act.

Psychology Progress Report
Upload the fillable PDF once and Vero Documents fills it from the visit note and the chart, ready for you to review and sign. Download, print, or email the finished form, with the patient’s consent.
A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

Only on request. ICBC's psychologist page marks the CL489Q as complete upon request, and the psychology program guide says progress reports must only be completed when an ICBC Claims Contact asks. ICBC funds the report only in that case.
The progress report focuses on change: it asks how symptom intensity or severity and functional status have changed since the last assessment, and replaces the initial treatment goals with updated goals and an updated plan. Both reports include a working diagnosis and a suicide risk item marked present or denied.
