Registered psychologists treating ICBC customers submit this six-page report once the initial assessment is complete. It covers work status, chief complaints, functional impact, how symptoms began and progressed, observed findings, suicide risk, psychological measures, a working diagnosis, medications and treatment goals. The Initial Visit and Report fee covers the assessment, one standard treatment and this report.
The psychologist completes and certifies the whole report and confirms either client consent or an ICBC request under section 28 or 28.1 of the Insurance (Vehicle) Act.

Psychology Initial Report
Upload the fillable PDF once and Vero Documents fills it from the visit note and the chart, ready for you to review and sign. Download, print, or email the finished form, with the patient’s consent.
A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

Up to four. ICBC's psychology program guide says the initial assessment may take up to four sessions, but the Initial Visit and Report fee is billed only once per customer unless ICBC approves otherwise. ICBC's psychologist page says visits beyond the first are billable at the standard treatment fee.
When a customer needs care beyond the Early Access Period, or recovery is not progressing as expected, the psychologist submits a Treatment Plan through HCPIR or HCPP. ICBC's guide recommends submitting it one to two weeks before the approved treatments or approved end date expire.
