Occupational therapists use this five-page form to recommend vehicle adaptations, or a vehicle acquisition, so an ICBC customer with a crash-related disability can travel as a driver or passenger. If the customer drives, it is completed only after a Functional Driver Evaluation and only when the Claims Contact requests it. Each recommended adaptation needs a functional or objective rationale.
An approved occupational therapist completes the whole request, including the nature of the disability, mobility aids, current vehicle, recommendations and any wheelchair details, and certifies it.

Vehicle Adaptation or Acquisition Request
Upload the fillable PDF once and Vero Documents fills it from the visit note and the chart, ready for you to review and sign. Download, print, or email the finished form, with the patient’s consent.
A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

Only when the ICBC Claims Contact requests it. If the customer is a driver, the form says to complete it only after a Functional Driver Evaluation, and the evaluation's outcome goes into the reason for referral. The request also states whether the adaptation is for the customer as a driver or a passenger.
No. The form says that once ICBC receives and reviews it, the Claims Contact will discuss next steps, and quotes should not be sourced until or unless written approval to proceed has been received. Costs above the approved amount are the customer's responsibility.
