ICBC CL794: Functional Job Match Assessment

Occupational therapists use this four-page template to compare an ICBC customer's demonstrated physical abilities with the critical demands of their job. Its stated purpose is baseline functional information for job match, not testing the client's maximum. Each capacity area, from sitting and walking to lifting and carrying, is marked as job demands met, not met or needing further assessment.

Who completes it

An approved occupational therapist completes the whole assessment, testing physical skills according to the client's diagnosis and identifying other functional barriers to returning to work.

Submitting and payment

How to submit it
Approved OTs submit it through the Health Care Provider Portal (HCPP) within five business days of the first meeting with the customer, after an assessment held within five business days of accepting the request.
Who pays
ICBC funds up to six hours for a Functional Job Match Assessment, including up to four hours of assessment and two of report writing, at the Enhanced Accident Benefit Regulation hourly rate.

Good to know

  • The assessment must be done at a facility with standardized functional assessment equipment, so ICBC funds no travel, mileage or gym fees for it.
  • It is a single service that ICBC requests, and the program guide says it concludes when the assessment is complete and the report is submitted.
  • Reported job demands should list weights, heights, distances and frequencies, with N/A entered where a demand is not required.
  • The report ends with up to four primary functional barriers to return to work and recommendations to address them.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

Frequently asked questions

What does the ICBC CL794 test?

Sitting, static and dynamic standing, walking, postural tolerances, stairs, reaching, hand dexterity, lifting, carrying, pushing and pulling, plus other job-specific tasks. For each area, the OT records the reported job demand and objective findings, then marks whether the demand is met, not met, or needs further assessment.

Is the CL794 the same as a functional capacity evaluation?

No. ICBC's OT program guide treats them as separate services: the job match assessment is capped at six hours, while a functional capacity evaluation can take up to 20 hours and requires five years of FCE experience or a Certified Work Capacity Evaluator designation.
