Occupational therapists in ICBC's Recovery Network use this 13-page report after the initial assessment of an ICBC customer, once ICBC has authorized the engagement. It documents medical and rehabilitation information, physical, cognitive and psychosocial limitations, daily function, work status and barriers, then sets up to five goals with a cost projection for the OT program. It is due within five business days of the first meeting.
The approved occupational therapist who assessed the customer completes and certifies the whole report. The OT confirms either client consent or that the report responds to an ICBC request under section 28 or 28.1 of the Insurance (Vehicle) Act.

Occupational Therapy Initial Report
Upload the fillable PDF once and Vero Documents fills it from the visit note and the chart, ready for you to review and sign. Download, print, or email the finished form, with the patient’s consent.
A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

ICBC's Occupational Therapy program guide, effective April 1, 2026, sets the deadline at five business days from the first meeting with the customer, unless the customer refuses consent or the ICBC Claims Contact sets another date in writing. The assessment itself must happen within five business days of accepting the service request.
Page 12 asks for the start and end of the OT program, estimated OT professional hours, rehabilitation assistant visits per week, hours per visit and weeks, and any expense items the OT purchases directly, noting whether each is pre-approved. Total approved OT hours also go on the ICBC OT Treatment Plan form.
