Occupational therapists close an ICBC engagement with this eight-page report. It counts total client visits, split between OT visits and rehabilitation assistant or kinesiologist visits, and gives updated medical status, limitations, daily function and work status, a summary and analysis, and return-to-function and return-to-work recommendations. ICBC expects it within five business days after the engagement ends.
The approved occupational therapist who led the engagement completes and certifies the whole report, confirming client consent or an ICBC request under section 28 or 28.1 of the Insurance (Vehicle) Act.

Occupational Therapy Discharge Report
Upload the fillable PDF once and Vero Documents fills it from the visit note and the chart, ready for you to review and sign. Download, print, or email the finished form, with the patient’s consent.
A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

ICBC's Occupational Therapy program guide, effective April 1, 2026, requires the discharge report within five business days of the termination of the engagement. The deadline does not apply if the customer's refusal to consent prevents release of the report or the ICBC Claims Contact sets a different date in writing.
It counts the total number of client visits, separating OT visits from rehabilitation assistant or kinesiologist visits, and lists dates of treatment since the last report. It also drops the OT Program Cost Projection, ending instead with a summary and analysis, recommendations and report distribution.
