ICBC CL797: Functional Driver Evaluation

Occupational therapists use this nine-page template to evaluate whether crash injuries affect an ICBC customer's functional ability to drive. It covers the driving profile, physical, cognitive, vision and behavioural testing, and an on-road evaluation for the licence class, then ends with one of four recommendations on driving rehabilitation and vehicle adaptations. It is separate from driver's licence medical fitness forms.

Who completes it

An approved occupational therapist completes the whole evaluation and certifies it; the form warns that false or misleading information can lead ICBC to cancel the vendor number.

Submitting and payment

How to submit it
Approved OTs submit it through the Health Care Provider Portal (HCPP) within five business days of the first meeting with the customer, unless the ICBC Claims Contact sets another date in writing.
Who pays
ICBC funds up to 11 hours for the evaluation, including up to seven hours of direct, communication and on-road time and four of analysis and report writing, plus up to 1.5 hours for initial file setup.

Good to know

  • The form must be completed in full, because incomplete reports may delay or affect treatment funding approval.
  • If driving rehabilitation is recommended, a table sets out the program dates, OT hours and driving school services; driving schools invoice ICBC directly.
  • For high-tech vehicle adaptations, the form asks you to confirm with the Claims Contact whether the separate adaptation or acquisition request (CL797A) is needed.
  • The on-road evaluation records the licence class tested, such as Class 5, 7N or 7L, and any adaptive driving equipment trialled.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

Frequently asked questions

What are the possible outcomes of an ICBC functional driver evaluation?

The summary offers four: no significant impairments affecting driving; impairments with driving rehabilitation recommended without vehicle adaptations; impairments with driving rehabilitation and vehicle adaptations recommended; or impairments where rehabilitation, modifications or vehicle acquisition are not expected to help at this time.

Is the CL797 a medical fitness to drive report?

No. It is an occupational therapy evaluation, requested by ICBC as a single service, of how crash-related impairments affect a customer's functional ability to drive. Driver's licence medical fitness reports are different forms with a different purpose.
