BC medical forms for British Columbia clinicians

All 84 verified BC forms, from WorkSafeBC and ICBC reports to SDPR disability assistance, PharmaCare Special Authority and RoadSafetyBC, with who completes each.

British Columbia has 84 verified forms in this library, all in English. WorkSafeBC accounts for 28, led by the Physician's Report (Form 8/11): Form 8 for the first visit, Form 11 for progress. ICBC adds 22, including the family physician's CL489 Standard and CL489A Extended Medical Reports. For disability assistance, the PWD Designation Application (HR2883) needs a Medical Report and an Assessor Report. PharmaCare coverage starts with a Special Authority request (HLTH 5328 or a drug-specific form).

Form 8 is often due within 3 business days of the first visit, and Teleplan pays the higher rate. ICBC physician reports are due within 30 days and can go through EMR-integrated versions or ICBC's online tool. Send Special Authority requests through PharmaCare eForms, not fax. RoadSafetyBC issues the DMER to the driver pre-populated, so the sample PDF isn't accepted. Vero Documents fills PDF forms from the visit note and chart; you review and sign. Federal forms (T2201, CPP disability, EI) apply here too and are on the federal hub.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

Vero is not affiliated with, or endorsed by, the agencies and insurers listed here. Read our editorial policy.

Frequently asked questions

Should I complete ICBC's CL489 or CL489A?

One or the other, never both, after the initial assessment. The CL489 Standard Medical Report fits injuries that caused no missed work or school and no significant impairment of daily living. The CL489A Extended Medical Report covers injuries that caused missed or modified work or school, or significant impairment. If recovery stalls or the diagnosis or care plan changes, use the CL489B Reassessment Medical Report.

Who can complete the BC PWD Designation Application?

Section 2, the Medical Report, must come from a medical practitioner or nurse practitioner. Section 3, the Assessor Report, can be done by a physician, NP or another prescribed professional: a registered nurse or psychiatric nurse, OT, PT, psychologist, chiropractor or registered social worker. Different professionals may complete the two reports. Physicians bill MSP for each section, and NPs and other professionals invoice SDPR.

How do I report a medically unfit driver in BC?

If RoadSafetyBC hasn't asked for a DMER, use the Report of a Condition Affecting Fitness and Ability to Drive (MV2351). Physicians, NPs, optometrists and psychologists can file it. Reporting is mandatory under section 230 of the Motor Vehicle Act when the patient keeps driving after being warned. RoadSafetyBC's guidance page says to send it by fax.

When is a WorkSafeBC Form 11 due?

Send Form 11 when the worker's condition, treatment or return-to-work status changes, when recovery is slower than the norms, or when more than 4 weeks have passed since the last one. Only one Form 8 is allowed per claim. WorkSafeBC pays its maximum only for prompt reports and nothing after 6 business days, and Teleplan reports pay more than fax or mail.
