BC HR2138: Medical Equipment Request & Justification

B.C.'s Ministry of Social Development and Poverty Reduction uses this two-page form to decide whether an assistance client can get ministry-funded medical equipment, which must be the most basic, least costly option meeting a medically essential need. A ministry worker starts it, a physician or nurse practitioner recommends the equipment, and an occupational, physical or respiratory therapist provides the detailed assessment.

Who completes it

The ministry worker completes and signs Section 1 with the client; the medical practitioner or nurse practitioner completes Section 2 (medical condition and equipment recommended); and an occupational, physical or respiratory therapist completes Section 3, certifying that the recommended equipment will meet the client's medical needs.

Submitting and payment

How to submit it
The completed form goes to the ministry's Health Assistance branch at the address printed on the form, which assesses all requests; equipment must be approved before it is purchased.

Good to know

  • No other resource, such as ICBC, WorkSafeBC, Veterans Affairs or private insurance, can be available to provide the equipment, and the worker confirms this in Section 1.
  • The therapist assessment covers what prompted the request, goals, diagnosis and prognosis, height and weight, function, physical and cognitive skills, environment, and current equipment with its repair history.
  • Product parameters need medical justification, including any components considered up-charges, and the assessment documents equipment trials and why options were eliminated.
  • Replacement periods include 5 years for wheelchairs, scooters, hospital beds and lifts and 2 years for wheelchair seating, unless a change in condition or growth requires sooner.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

Frequently asked questions

Who completes the BC Medical Equipment Request and Justification?

Three parties: a ministry worker completes Section 1 with the client, who signs the consent; a medical practitioner or nurse practitioner recommends the equipment in Section 2; and an occupational, physical or respiratory therapist gives the detailed specifications and functional assessment in Section 3.

Will the ministry pay for equipment bought before approval?

No. Ministry policy says medical equipment must be pre-approved before purchase, and without prior approval the ministry will not pay, except in a life-threatening emergency. The item must also be the least expensive appropriate option for the medically essential need.
