BC Form HR2847: Monthly Nutritional Supplement (MNS)

B.C. disability assistance recipients with the Persons with Disabilities designation use this application to request the Monthly Nutritional Supplement, which covers vitamins or minerals and extra nutritional items. It is for people treated for a chronic, progressive deterioration of health due to a severe medical condition, with at least two qualifying symptoms. The items must alleviate those symptoms and prevent imminent danger to life.

Who completes it

A medical practitioner, nurse practitioner or registered dietitian completes Part C: the severe conditions, the treated progressive deterioration, symptoms, height and weight, and the vitamins, minerals or nutritional items needed with justification. Ministry staff complete Part A and the applicant signs Part B.

Submitting and payment

How to submit it
The recipient returns the completed form to their Employment and Assistance Office, which checks it and forwards it to Health Assistance for adjudication; Health Assistance notifies the client and the office.
Who pays
Medical practitioners may bill MSP fee item 96400 for completing Part C; the form notes this billing is for medical practitioners only.

Good to know

  • Qualifying symptoms are malnutrition, underweight status, significant weight loss, significant muscle mass loss, significant neurological degeneration, significant vital organ deterioration, and moderate to severe immune suppression.
  • Do not complete it if supplements are needed for 3 months or less, or if a diet supplement such as high protein or gluten-free would meet the need.
  • Vitamins and minerals exclude homeopathic, naturopathic and herbal remedies, and nutritional items must provide caloric supplementation to a regular dietary intake.
  • Health Assistance reviews eligibility periodically and may ask for a new HR2847; if the recipient does not return it on time, the supplement ends at the review date.
