BC HLTH 2810: Out-of-Country Health Services Funding

BC specialists use this 2-page application to request MSP prior approval before a patient receives elective, non-laboratory treatment outside Canada. Without pre-approval, all costs of care received outside Canada fall to the patient, even when the care is urgent. MSP funds only treatment that would be insured in BC and that is unavailable in Canada or needed abroad to avoid harmful delay.

Who completes it

The attending BC specialist completes and signs the whole form; without their signature it is considered incomplete. MSP expects a practitioner actively involved in the patient's care in BC with expert knowledge of the proposed service.

Submitting and payment

How to submit it
Send the form and supporting medical documentation to Provider Programs by mail or fax, as printed on the form, noting any urgency. Urgent applications go by fax to Health Insurance BC's out-of-country claims team with a number where the specialist can be reached.

Good to know

  • Attach consultation reports and recommendations from the other BC and Canadian specialists consulted. Cancer treatment requests need a copy of the BC Cancer Agency's medical recommendation.
  • Page 2 asks whether treatment is available in BC or elsewhere in Canada, whether it is experimental, and whether a delay would cause death or medically significant irreversible tissue damage.
  • MSP does not pay travel, accommodation, take-home or outpatient drugs, or services from non-physician practitioners, and submitting the form does not guarantee funding.
  • Out-of-country laboratory and genetic testing go through BC's Agency for Pathology and Laboratory Medicine instead of this form.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

Frequently asked questions

Who can sign the BC Out-of-Country Health Services Funding Application?

Only the attending BC specialist. The form states that it must be completed by the attending BC specialist and include their signature, or it is considered incomplete. MSP describes this as a practitioner actively involved in the patient's care in BC who has expert knowledge of the proposed service or specialty.

What criteria does MSP apply to out-of-country treatment requests?

Treatment must be medically required, insured if done in BC, given by a licensed physician or oral surgeon in an accredited hospital, and generally accepted by BC's medical profession. It must be unavailable in Canada, or a delay must risk medically significant irreversible consequences. Clinical trials and experimental or research-stage services are excluded.

What if the patient needs life-saving care after hours?

For immediate, life-saving care that requires acute hospital admission outside regular hours, MSP directs providers to the BC Patient Transfer Network, which manages medical transfers within and outside BC. Other urgent applications are faxed to Health Insurance BC's out-of-country claims team with a contact number for the referring specialist.
