TAP BC Request for Travel Assistance (HLTH 2900)

BC's Travel Assistance Program form lets an MSP patient referred to an insured specialist service outside their community get waived or discounted fares from partner carriers such as airlines, BC Ferries, bus and rail. The referring physician or nurse practitioner issues the sequentially numbered form at the time of referral. TAP does not pay patients directly or cover meals, lodging or fuel.

Who completes it

The referring physician or nurse practitioner's office completes Part One (referring and destination practitioners, destination city, appointment date, Escort Required box) and signs or stamps it. The patient adds their address and signs the declaration, and hospital staff use Part Two only for a return home after arrival by ambulance.

Submitting and payment

How to submit it
Give the form to the patient at referral; the patient calls TAP for a confirmation number before booking, ideally about two weeks ahead, and presents the form to the carrier. Offices order the forms from Health Insurance BC by automated phone line or fax using their MSP practitioner number.

Good to know

  • Escorts qualify only for patients aged 18 or under or medically unable to travel alone, and the physician or nurse practitioner must mark Escort Required on the form.
  • Issue one form per referral. Each new appointment, even in the same course of treatment, needs a new form from the destination office and a new confirmation number.
  • Forms stay valid for 90 days after the appointment date; if the appointment moves by more than 90 days, issue a new form. Carriers may refuse altered or incomplete forms.
  • GP-to-GP and NP-to-GP referrals, transfers between facilities, and visits to practitioners such as physiotherapists or chiropractors are not eligible for TAP.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

Vero is not affiliated with, or endorsed by, the agencies and insurers listed here. Read our editorial policy.

Frequently asked questions

How does a clinic get TAP BC forms?

The forms can't be downloaded. Physicians and nurse practitioners order the pre-printed, sequentially numbered forms from Health Insurance BC through an automated telephone line using their MSP practitioner number, or by fax with their name, full mailing address and practitioner number. Hospital social work departments and specialty clinics can also give forms to eligible patients.

Who is eligible for the Travel Assistance Program?

BC residents enrolled in MSP who are referred by a physician or nurse practitioner to MSP-insured, non-emergency specialist services that aren't available in their community, at the closest appropriate location. Travel costs must not be covered by another payer, such as an extended health plan, ICBC, WorkSafeBC or a federal program.

Does TAP BC pay for the patient's trip?

Not directly. TAP is a partnership in which carriers waive or discount regular fares when the patient presents the form with a confirmation number. It doesn't pay patients, book travel, or cover meals, accommodation, mileage, fuel or local transportation, and nothing is reimbursed after travel.
