BC Special Authority for Dexcom and Libre (HLTH 5817)

Prescribers use this two-page Special Authority request to get BC PharmaCare coverage of a Dexcom G6 or G7 or a FreeStyle Libre 2 or 3 Plus glucose monitoring system. Initial approval runs three years and needs intensive insulin therapy, a commitment to diabetes education and a qualifying reason such as hypoglycemia unawareness; renewals run five years.

Who completes it

The prescriber completes sections 1 to 3 for initial coverage, or sections 1, 2 and 4 for renewal, and signs to confirm they discussed the release of information with the patient.

Submitting and payment

How to submit it
PharmaCare prefers Special Authority eForms, which return decisions faster; the PDF can also be faxed or mailed to PharmaCare, and faxed requests currently take about five weeks.

Good to know

  • Intensive insulin therapy means an insulin pump or at least 4 daily injections: 1 or more basal and 3 or more bolus.
  • Frequent hypoglycemia means more than 3 episodes a week; unpredictable glucose swings mean readings below 4 or above 10 mmol/L without a predictable cause.
  • Insulin users with a CGM or flash monitor are limited to 200 blood glucose test strips a year, though children under Nursing Support Services may need more.
  • Age limits: Dexcom G6 or G7 and FreeStyle Libre 3 Plus from age 2, and FreeStyle Libre 2 from age 4.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

Frequently asked questions

Who qualifies for Dexcom or Libre coverage under BC PharmaCare?

Initial criteria need diabetes in an eligible age group, insulin pump therapy or multiple daily injections, agreement to diabetes education and regular follow-up, and at least one of: hypoglycemia unawareness, frequent or unpredictable hypoglycemia, unpredictable glucose swings, a functional restriction on test strip use, or a job where hypoglycemia is a safety risk.

How long does BC PharmaCare glucose monitor approval last?

Initial coverage under section 3 lasts three years. Renewal under section 4 lasts five years and requires the same insulin therapy criteria plus the prescriber's confirmation that the patient will benefit from continued use of the glucose monitoring system.

Why would a glucose monitor Special Authority be returned?

PharmaCare returns forms with missing information for completion, and cannot send a response if no prescriber fax number or mailing address is given. The form marks the prescriber's fax number as critical for a timely response and the patient's Personal Health Number as critical for processing, and the signature is mandatory.
