BC Palliative Care Benefits Registration (HLTH 349)

A physician or nurse practitioner uses this three-page registration to enrol a palliative patient in PharmaCare Plan P, which covers formulary prescription and over-the-counter medications, and to refer them to home and community care for palliative supplies and equipment. The practitioner records indicators of deteriorating health and life-limiting illness and certifies that the patient meets all four eligibility criteria.

Who completes it

The medical or nurse practitioner completes Steps 1 to 5, certifying eligibility with their CPSID or PharmaNet ID; the patient, or a legal representative or the practitioner if the patient cannot sign, signs the Step 6 consent.

Submitting and payment

How to submit it
Fax pages 1 and 2 to Health Insurance BC for drug coverage and to the local home and community care office for the equipment assessment, sending it only once to each.

Good to know

  • Eligibility requires a life-threatening illness, life expectancy of up to 6 months, consent to a palliative rather than curative focus, and wishing to receive palliative care at home.
  • 'Home' can be the patient's own home, a family or friend's home, assisted living, or a residential care facility such as a community hospice bed not covered by Plan B.
  • A reassessment is required after 12 months, and the same form cancels the registration when the patient no longer qualifies.
  • Being on Plan P is also a prescribed class for B.C.'s Persons with Disabilities (PWD) designation.

Frequently asked questions

What does BC PharmaCare Plan P cover?

Plan P covers 100% of the eligible cost of prescription and over-the-counter medications in the Plan P formulary, so practitioners must prescribe formulary items for coverage. Medications outside the formulary may be covered under the patient's usual PharmaCare plan, such as Fair PharmaCare or Plan C.

How quickly does palliative drug coverage start in BC?

Drug coverage begins as soon as the registration is processed, normally within 24 hours. Coverage of palliative medical supplies and equipment begins later, after a health professional from the local home and community care office has contacted the patient and assessed their needs.

What clinical indicators does the palliative registration require?

Step 2 needs two or more general indicators of poor or deteriorating health, such as unplanned admissions or progressive weight loss, drawn from SPICT. Step 3 needs at least one clinical indicator of a life-limiting condition in cancer, heart, dementia or frailty, neurological, kidney, liver, respiratory or other disease.
