BC PharmaCare PPI Special Authority (HLTH 5350)

This one-page form, part of BC PharmaCare's Reference Drug Program, is how a prescriber requests Special Authority coverage of a proton pump inhibitor. The prescriber checks the diagnosis, such as GERD, a peptic ulcer, Barrett's esophagus or H. pylori eradication, then requests a fully covered reference PPI, full coverage of a partially covered PPI after failed trials, or a compounded suspension.

Who completes it

The prescriber completes sections 1 to 3 and one of sections 4, 5 or 6, then signs to confirm the patient was told why their information is released to PharmaCare.

Submitting and payment

How to submit it
Submit through PharmaCare's Special Authority eForms, or fax or mail the PDF to PharmaCare as printed in its header; faxed requests currently take about five weeks.

Good to know

  • Full coverage of esomeprazole, lansoprazole, omeprazole or pantoprazole sodium requires failed trials of at least 4 weeks of both rabeprazole and pantoprazole magnesium at adequate doses.
  • H. pylori eradication as part of triple therapy is approved for a maximum of 14 days.
  • A compounded omeprazole or lansoprazole suspension needs a reason, such as being unable to use capsules, water-dispersible or orally disintegrating forms, or a dose not commercially available.
  • Forms missing information are returned for completion, and without a prescriber fax number or mailing address PharmaCare cannot send a response.
