HBT Form C: APS of Continuing Disability (LTD)

Once a Healthcare Benefit Trust long-term disability claim is underway, the attending physician provides periodic updates on Form C. It is one of HBT's LTD maintenance forms, alongside the claimant's Form A and the Form D statement, and like the initial statement it goes to Canada Life, the Trust's claims-paying agent. The current revision is dated April 2026.

Who completes it

The claimant signs the authorization releasing claim information to Canada Life, the Trustees of the Trust and their agents. The attending physician completes the seven sections, from diagnosis to additional information and remarks.

Submitting and payment

How to submit it
The physician chooses how to return it: give it to the patient, mail it to Canada Life, or email it to the Canada Life Disability Management Services office in Calgary, Langley or Vancouver.
Who pays
The form states that the patient is responsible for any charges made for its completion.

Good to know

  • Section 2 asks for complications, recent surgery or new independent conditions contributing to the duration of disability, and whether the patient is ambulatory or bed, house or hospital confined.
  • Section 6 asks when the patient can return to their own occupation or seek other employment, full-time or part-time, or an estimate in weeks or months if indefinite.
  • Section 7 is a checklist showing whether consultation reports, lab tests, diagnostic results, clinical notes and other reports are attached or unavailable.
  • Section 3 asks for the American Heart Association functional class and latest blood pressure when a cardiac condition is involved.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

Frequently asked questions

What is the difference between HBT's APS and Form C?

The Attending Physician's Statement supports the initial long-term disability claim, while Form C, the Attending Physician's Statement of Continuing Disability, updates an ongoing claim. Form C focuses on progress since the last report: recovered, improved, not improved or retrogressed, current treatment, and return-to-work prognosis.

Does Form C ask about rehabilitation?

Yes. Section 6 asks whether the services of a rehabilitation consultant would help the patient return to work, and whether the patient is a suitable candidate for some form of trial employment or retraining, with space to comment.

Can the patient see the completed Form C?

The form states that copies of it and other medical reports will be released to the patient when requested in writing by the patient. Physicians can use the back page to expand on any heading or add information relevant to the claim.
