Sun Life Initial Disability Medical Statement (5548)

Sun Life uses its copy of the industry Initial Disability Insurance Medical Statement, form 5548-SL-IDIMS-E, for new short-term disability, long-term disability and salary continuance claims. It replaces Sun Life's own Attending Physician's Statement for new claims, although older statements such as 045-1355 still appear inside some legacy claim packages. Sun Life's packages pair it with plan member and plan sponsor statements.

Who completes it

Section 2, the Medical Statement, is completed by the attending physician or other medical provider. The plan member and plan sponsor complete their own statements in Sun Life's claim packages.

Submitting and payment

How to submit it
The statement is faxed or mailed to the Sun Life Group Disability Management office handling the claim.
Who pays
Sun Life's claim guide says the patient pays any fee the doctor charges for the statement.

Good to know

  • The same statement serves short-term disability, long-term disability and salary continuance claims, so the form a patient brings does not depend on the benefit type.
  • A French version, Déclaration médicale initiale pour l'assurance invalidité (5548-SL-IDIMS-F), is posted on Sun Life's French group forms page.
  • The industry statement's Section 2 covers diagnosis, treatment and response, hospitalization, investigations, symptoms, restrictions and limitations, prognosis and return-to-work goals.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.
