WorkSafeBC Form 83D665: Physiotherapy Treatment Report

Physiotherapists contracted with WorkSafeBC for musculoskeletal care use this four-page report for two purposes: a treatment plan request (83D219) to extend care, or a discharge report (83D222) after the last visit. It covers return-to-work status and next steps, changes in subjective and objective findings, functional abilities, goals, outcome measures, and a request summary with visits and hydrotherapy.

Who completes it

The treating physical therapist completes the whole report and confirms whether the findings and treatment goals were discussed with the worker.

Submitting and payment

How to submit it
Upload it through WorkSafeBC's claims uploader if you invoice in Teleplan, or submit it in My Provider Services, where treatment plan requests are tracked online.
Who pays
WorkSafeBC pays under its physiotherapy fee schedules, and the invoice's date of service must match the form: the submission date for a plan request, or the last visit for a discharge.

Good to know

  • If the worker has not returned to work, recommend further physiotherapy, an interdisciplinary program with supporting rationale, or an RTW plan with an anticipated date.
  • Report only significant changes in findings, improvements or deterioration, since the intake assessment or the last treatment plan report.
  • Complete two outcome measures from NPRS, RMDQ, NDI, LEFS, DASH or UEFI, plus either the Pain Disability Index or the Örebro questionnaire.
  • The request summary asks whether hydrotherapy is provided or requested, and whether the worker has a home exercise program and education on active pain management.

Frequently asked questions

How do I request more physiotherapy visits from WorkSafeBC?

Complete the Physiotherapy Treatment Report as a treatment plan request (83D219). The request summary gives the initial assessment date, visits to date, number of visits requested and requested end date, with treatment goals for the extension, two outcome measures, and the PDI or Örebro questionnaire.

Is the WorkSafeBC physiotherapy discharge report a separate form?

No. The discharge report (83D222) is a report type on form 83D665; tick it and use the date of the last visit as the date of service. If the worker has returned to work and needs no further treatment, enter the discharge date in the return-to-work section.
