Contracted Comprehensive Mental Health Program (CMHP) teams use this report at three points: the intake report (83D683), the core program summary after the 10-week treatment period (83D684), and the continued intervention summary at discharge (83D685). It covers participation, clinical summary and outcome measures, return-to-work strengths and risks, workplace factors, self-harm risk, and discharge status and recommendations.
The treating occupational therapist and treating mental health clinician (psychologist, registered clinical counsellor or registered clinical social worker) prepare the report and list the assessment or treatment team.
