WorkSafeBC Form 83D487: OT Services Progress Report

Occupational therapists contracted under WorkSafeBC's Occupational Therapy Services program use this report to update the claim every 8 weeks, unless a WorkSafeBC officer sets a different schedule. It covers subjective reports, objective findings, basic and instrumental activities of daily living, vocational status, home environment and equipment, an analysis of progress, and the plan and sessions requested for the next treatment block.

Who completes it

The contracted occupational therapist completes and signs the report, declaring it true and accurate, and ticks the stream: physical injury (OT-PI), mental health (OT-MH) or brain injury (OT-BI).

Submitting and payment

How to submit it
Submit it through WorkSafeBC's Provider Portal after discussing the proposed plan with the WorkSafeBC officer, recording the date of that communication and the officer's name.
Who pays
WorkSafeBC approves sessions by type, such as OT visit for treatment, rehabilitation worker visit or OT RTW support, with payment to the company's WorkSafeBC payee number.

Good to know

  • Goals must be SMART (specific, measurable, attainable, realistic and timely) and tied to the compensable injury and the recovery and return-to-work factors identified.
  • OT RTW Support replaces OT Visit for treatment for all in-person services provided during a graduated return to work.
  • If continued equipment rental is recommended, state the expected timeframe, and explain why if the rental date has passed and the equipment is still needed.
  • Related forms include the Occupational Therapy Services Discharge Report (83D486), the GRTW Plan (83D202) and the Job Site Visit Report (83D201).

Frequently asked questions

How often is the WorkSafeBC OT progress report due?

Every 8 weeks, unless a WorkSafeBC officer indicates otherwise. Each report gives the end date of the next treatment block, a proposed discharge date and the number of sessions requested, and records the progress report number, total sessions approved and the history of sessions attended since services began.

Do I need to talk to the WorkSafeBC officer before submitting the OT plan?

Yes. The form says the provider should communicate the proposed plan to the WorkSafeBC officer before submitting the report. It asks for the date of that communication, the officer's name, and comments such as whether the plan was approved or declined and whether contact was by phone call or voice message.
