Ontario OCF forms: OCF-18, OCF-23, OCF-3 and more

The FSRA accident benefits forms Ontario health practitioners complete for auto insurers, from treatment plans to disability and catastrophic impairment reports.

Ontario auto insurers handle accident benefits under the SABS with standard OCF forms published by FSRA. Health practitioners complete the Treatment and Assessment Plan (OCF-18) for care outside the Minor Injury Guideline, the Treatment Confirmation Form (OCF-23) for injuries within it, the Disability Certificate (OCF-3) for income replacement, non-earner or caregiver benefits, and the Application for Determination of Catastrophic Impairment (OCF-19). Assessors use Form 1 to document attendant care needs.

HCAI-registered facilities submit the OCF-18, OCF-23 and OCF-21 invoice through HCAI, and other providers send them to the insurer. The OCF-3 and OCF-19 go back to the applicant's auto insurer, and their completion fees are billed to the insurer. Every form here is fillable and bilingual, and all were re-issued on July 1, 2026 with the SABS changes, so check you have the current version. With Vero Documents, any fillable PDF you upload becomes a template the whole clinic can use.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

Frequently asked questions

What is the difference between the OCF-18 and the OCF-23?

The OCF-23 Treatment Confirmation Form is for impairments within the Minor Injury Guideline (MIG): the initiating health practitioner confirms the injury fits so pre-approved MIG treatment can start. The OCF-18 Treatment and Assessment Plan is for treatment, assessments or goods and services outside the MIG, and the insurer must approve it. When MIG treatment ends, the practitioner completes the OCF-24 discharge report.

Where do I get the OCF-18 and how is it submitted?

FSRA publishes the OCF-18 as a fillable PDF in English and French. HCAI-registered facilities submit it through HCAI (Health Claims for Auto Insurance), and the form has a field for the HCAI Facility Registry Number. Other providers send it to the insurer. The health practitioner completes Part 4 and the regulated health professional Part 5, and they may be the same person. Approved services are billed on the OCF-21.

Who can complete the OCF-19?

A physician completes the Application for Determination of Catastrophic Impairment (OCF-19), or a neuropsychologist when the impairment is a brain impairment only. The applicant or a substitute decision maker completes Part 1, and the clinician completes Parts 2 to 5, including the Section A or B criteria. It goes to the applicant's auto insurer. It is not an OHIP-insured service, so the completion fee and examination costs are billed to the insurer.

Who completes the OCF-3 Disability Certificate?

A health practitioner, as defined in the SABS, completes Parts 5 to 10: injuries and sequelae with ICD-10-CA codes, disability tests, investigations and consultations, prior and concurrent conditions, medications and signature. The applicant completes Parts 1 to 4. The certificate supports income replacement, non-earner or caregiver benefits and goes back to the applicant's auto insurer. The completion fee is billed to the insurer directly.
