OCF-21: Auto Insurance Standard Invoice (Ontario)

Ontario auto insurers are billed on this standard invoice for assessments, treatment and goods they have approved under the accident benefits rules (SABS). It comes in two versions: Version B for services outside the Minor Injury Guideline, approved on an OCF-18 Treatment and Assessment Plan, and Version C for Minor Injury Guideline (MIG) treatment started with an OCF-23 Treatment Confirmation Form.

Who completes it

The health care provider or facility that delivered the approved services completes the whole invoice: Parts 1 to 4 plus the Version B or Version C service-detail pages.

Submitting and payment

How to submit it
HCAI-registered facilities submit it electronically through HCAI (Health Claims for Auto Insurance), where it appears as OCF-21B or OCF-21C; other providers send the invoice directly to the auto insurer.
Who pays
The auto insurer pays the provider for the services it approved on the OCF-18 or OCF-23, and Version C shows the MIG reimbursable-fee totals.

Good to know

  • Each invoice rests on an approval: the OCF-18 for services outside the MIG, or the OCF-23 for treatment within the Minor Injury Guideline.
  • When MIG treatment ends, the practitioner also completes the OCF-24 Minor Injury Treatment Discharge Report, which goes with Version C billing.
  • FSRA's forms table shows the OCF-21 (AF-134E) as last updated July 1, 2026, when it re-issued the SABS claim forms with that day's SABS changes.
  • The PDF can be filled in and saved, and FSRA publishes it in English and in French as the Facture d'assurance-automobile standard.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

Frequently asked questions

What is the difference between OCF-21 Version B and Version C?

Version B bills services outside the Minor Injury Guideline that the insurer approved on an OCF-18 Treatment and Assessment Plan. Version C bills Minor Injury Guideline treatment started with an OCF-23 Treatment Confirmation Form and includes the MIG reimbursable-fee totals. Both share Parts 1 to 4; only the service-detail pages differ.

How is the OCF-21 submitted?

HCAI-registered facilities submit the invoice electronically through HCAI (Health Claims for Auto Insurance), where it appears as OCF-21B or OCF-21C. Providers outside HCAI send the completed invoice directly to the auto insurer. In both cases, the invoice should match the services the insurer approved on an OCF-18 or OCF-23.

Is the OCF-21 available in French?

Yes. FSRA publishes the Auto Insurance Standard Invoice in English and in French, as the Facture d'assurance-automobile standard. The PDF can be filled in and saved, and the version on FSRA's site carries the July 1, 2026 update made alongside that day's changes to Ontario's accident benefits rules.
