Ontario OCF-24: Minor Injury Treatment Discharge Report

Ontario health practitioners complete this two-page form when an insured person's treatment under the Minor Injury Guideline (MIG) ends. It reports discharge status and functional status at discharge to the auto insurer, closing the MIG episode that began with the OCF-23 Treatment Confirmation Form. It applies to accidents on or after September 1, 2010.

Who completes it

The health practitioner who provided the MIG treatment completes the whole form, including Part 3 (Health Practitioner Information and Signature), Part 4 (Discharge Status) and Part 5 (Functional Status at Discharge).

Submitting and payment

How to submit it
Send the completed form to the insurer. The OCF-24 is not on HCAI's provider-support form list, whereas the OCF-21 Version C invoice is an HCAI form.
Who pays
MIG services are billed to the insurer on the OCF-21 Version C, with block fees set in FSRA's Minor Injury Guideline.

Good to know

  • FSRA re-issued its Statutory Accident Benefits Schedule claim forms with the July 1, 2026 changes; the current OCF-24 carries version AF-123E (2026).
  • FSRA describes the PDF as fillable and saveable, so it can be completed and stored electronically before it goes to the insurer.
  • The OCF-23 starts pre-approved MIG treatment once the initiating practitioner confirms the impairment falls within the guideline; the OCF-24 is its discharge counterpart.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

Frequently asked questions

Who completes the OCF-24?

The health practitioner who provided treatment under the Minor Injury Guideline completes all five parts. Part 3 holds the practitioner's information and signature, Part 4 the discharge status and Part 5 the insured person's functional status at discharge.

Is the OCF-24 submitted through HCAI?

No. The OCF-24 is not on HCAI's provider-support form list, so it goes to the insurer. MIG invoicing is handled differently: the OCF-21 Version C used for MIG billing is an HCAI form, as is the OCF-21 Version B for non-MIG services.

Is there a French version of the OCF-24?

Yes. FSRA publishes the form in English and French; the French title is Rapport de congé de traitement aux fins de la Ligne directrice sur les blessures légères. FSRA publishes both language versions as fillable PDFs.
