Nova Scotia medical forms for physicians and NPs

All 37 verified Nova Scotia forms: WCB reports, Pharmacare exception status drug requests, ESIA and Disability Support Program reports, RMV medicals and student aid.

Nova Scotia has 37 verified forms in this library, all in English, and 22 come from WCB Nova Scotia. Physicians and NPs report work injuries on the Primary and Emergency Care Report at the first visit and update it at later visits. Drug coverage runs through Pharmacare's Request for Coverage of Exception Status Drug, plus 58 drug- and condition-specific forms. The RMV requires the Driver's Medical Examination Report for class 1 to 4 licences, and yearly after age 64.

WCB Nova Scotia routes sprain, strain, concussion and psychological injury care through its Work-Connected Recovery network, so its Forms B, C and E are for out-of-province providers. ESIA and Disability Support Program medical assessments come through the caseworker or care coordinator and aren't published online. Ocean pilot prescribers can submit drug requests online. Students upload the PSAS Verification of Disability to MyPATH. In Vero Documents, any fillable PDF you upload becomes a clinic-wide template. Federal forms (T2201, CPP disability, EI) apply here too; see the federal hub.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

Vero is not affiliated with, or endorsed by, the agencies and insurers listed here. Read our editorial policy.

Frequently asked questions

Which form do Nova Scotia physicians use for WCB injuries?

The Primary and Emergency Care Report, posted in November 2025, for physicians and NPs. It covers the incident, diagnosis, clinical notes, treatment and return-to-work barriers, and is updated at later visits. No Form 8/10 appears in WCB Nova Scotia's current provider library. Fax it to WCB, and bill through MSI billing software with WCB as payee within 6 months, using code WCB26 printed on the form.

Who uses WCB Nova Scotia Forms B, C and E?

WCB Nova Scotia lists Forms B (initial assessment), C (progress) and E (physical abilities) for out-of-province providers. In Nova Scotia, care for sprains, strains, concussion and psychological injury goes through the Work-Connected Recovery network, whose providers don't submit forms or invoices through WCB Online. Since January 1, 2026 their invoices go to Lifemark on the ACMS platform.

How do I request Nova Scotia Pharmacare coverage for an exception status drug?

Look for one of the 58 specialized forms first, for example ADHD agents, rheumatoid arthritis or sensor-based glucose monitoring. If none applies, use the standard Request for Coverage of Exception Status Drug. Fax or mail it to Pharmacare, or submit online through Ocean if you're in the ESD pilot. Forms may be added or changed at any time, so download a fresh copy each time.

Who certifies a Nova Scotia accessible parking permit?

A qualified medical practitioner certifies the disability on the Application for Accessible Parking Identification Permits and Plates (APP11): Section 2 for a permanent permit, or Section 3 for a temporary permit of up to 6 months. Permits are free, and applicants can self-certify a permanent-plate renewal in Section 4. The accessible motorcycle plate has its own form, which a licensed NP can also certify.
