WCB Nova Scotia Medical Cannabis Extension Request

Authorizers use this five-page form to ask WCB Nova Scotia to extend coverage of medical cannabis for a worker already approved, or to adjust the approved dosage. It records the latest assessment, weekly function and pain scores, whether either has improved since starting cannabis, side effects and risk concerns, new conditions or medication changes, and revised functional goals. Every request is evaluated against WCB's medical cannabis coverage criteria.

Who completes it

The authorizing practitioner who manages the worker's cannabis treatment completes the whole form and signs it with their licence number; a fax number is mandatory.

Submitting and payment

How to submit it
Fax or mail the form to Medavie Blue Cross; for a worker's first request, use the Initial Request Form for Medical Cannabis instead.
Who pays
Physicians bill fee code WCB30 through MSI for the completed form, and other prescribers invoice WCB directly.

Good to know

  • WCB's criteria call for follow-up at least every 12 weeks to assess efficacy and ongoing need, and for treatment to stop if there is no appreciable clinical effect or significant side effects occur.
  • Function and pain are scored 0 to 10 as weekly averages since the last assessment, where 0 is pre-injury function or no pain and 10 is severe impact or persistent severe pain.
  • Coverage limits: no smoking, no more than 3 grams of dried cannabis a day, THC no higher than 9%, and THC generally no more than 30 mg a day and never above 75 mg.
  • The risk review covers side effects, dependence, relapse of substance use, driving impairment, work performance, work site safety standards and the worker's ability to take part in rehabilitation.

Frequently asked questions

When do I use WCB Nova Scotia's medical cannabis extension form?

When a worker already has WCB-approved medical cannabis and you are asking to continue coverage or adjust the approved dosage. For a first request, use the Initial Request Form for Medical Cannabis. Either way, the request is evaluated against WCB's medical cannabis coverage criteria, which call for reassessment at least every 12 weeks.

What dosing details does the cannabis extension request need?

Grams per day and duration, the mode of administration (vaporized or oil), and for each product the strain or name, THC and CBD percentages, and the daily THC dose in mg. For dried cannabis the form gives the formula: daily quantity in mg multiplied by THC percentage as a decimal. Oils need concentrations and daily volume.

Does WCB Nova Scotia cover vaporizers for medical cannabis?

Where vaporizing is the approved route, WCB's medical cannabis guidelines say it will cover up to a reasonable cost of a vaporizer, with replacements limited to once every two years. Other devices and paraphernalia are not covered, and smoking is never a covered mode of administration.
