WCB Nova Scotia Physician Assistive Devices Request

WCB Nova Scotia uses this form when a physician recommends an assistive device for a worker's compensable injury. The case worker fills in the request and worker details, and the physician describes the device, whether it is new or a replacement, any leg length discrepancy if orthotics or footwear are prescribed, and the rationale and benefit in relation to the injury, with supporting evidence where needed.

Who completes it

The WCB case worker completes Sections A and B; the physician completes Section C on the device and Section D with their name, professional designation, WCB ID number, signature, phone and fax.

Submitting and payment

How to submit it
Fax the completed form back to WCB with the worker's claim number on every page; WCB lists the assistive devices request among the forms for approved out-of-province clinics.
Who pays
The form lists no fee code, and WCB says only services it has pre-authorized are guaranteed for payment.

Good to know

  • Explain the rationale and benefit of the device in relation to the compensable injury, and attach medical or scientific evidence where necessary to support the request.
  • If you prescribe orthotics or footwear, say whether the worker has a leg length discrepancy and indicate the discrepancy.
  • Sections A and B belong to the WCB case worker, covering the request details and the worker's name, date of birth, date of injury, compensable injury and body part injured.
  • Other providers, such as physiotherapists, chiropractors or orthotists, use a separate Service Provider version that also asks for device cost, warranty details and orthotics and footwear specifics.

What should the rationale for an assistive device include?

The form asks for the rationale and benefit of the device in relation to the compensable injury and, where necessary, medical or scientific evidence supporting the request. You also state the type of device, whether it is new or a replacement, and for orthotics or footwear whether there is a leg length discrepancy.
