WCB Nova Scotia CTS Physician Hand/Wrist Report

WCB Nova Scotia uses this three-page report to assess whether a worker's hand or wrist symptoms, such as potential carpal tunnel syndrome, are causally related to the workplace. It walks through symptom history, Phalen's, Tinel's and other examination findings, a diagram of where sensations occur, the diagnosis or differential, and conditions such as diabetes or thyroid disorders that can contribute.

Who completes it

The worker's family doctor must complete, sign and date the form, covering sections A to M and the health care provider block with their name, HS number, phone and fax.

Submitting and payment

How to submit it
Upload the completed PDF as a secure-message attachment in WCB Online, or fax it to WCB, with the WCB claim number on every page. Attach copies of any electrodiagnostic testing such as EMG or NCV.
Who pays
Physicians bill the report through MSI with WCB as the payee under fee code WCB20, the carpal tunnel syndrome assessment report, within six months of the service date.
