WCB Nova Scotia Medical Cannabis Initial Request

Prescribers use this seven-page form for a worker's first request for WCB Nova Scotia coverage of medical cannabis. It documents the diagnosis and objective findings, prior pharmaceutical trials including a synthetic cannabinoid, a risk assessment, current medications and substance use, a cannabis treatment agreement, baseline function and pain, measurable goals, and the proposed product and THC dose. Requests are judged against WCB's medical cannabis criteria.

Who completes it

The prescriber completes the whole form after an in-person physical exam and must agree to manage and monitor the condition being treated. If they are not the primary treating health professional, that professional must be aware of and support the treatment.

Submitting and payment

How to submit it
Fax or mail it to Medavie Blue Cross with the WCB claim number; requests to extend coverage or adjust the dose use the separate Extension Request Form.
Who pays
Physicians bill fee code WCB29 through MSI for the completed form, and other prescribers invoice WCB directly.

  • For refractory neuropathic pain, the worker must have failed adequate trials of three categories of first- or second-line agents, and requests are not considered without a failed synthetic cannabinoid trial.
  • Coverage is not considered if the worker is under 25, has a personal or strong family history of psychosis, a cannabis or active substance use disorder, cardiovascular or respiratory disease, or is pregnant or breastfeeding.
  • A cannabis treatment agreement, including instruction on concurrent recreational use, must be completed and acknowledged on the form before the request is processed; WCB posts sample agreements.
